IHSA SPORTS PHYSICALS ARE REQUIRED TO BE UPLOADED FOR ALL ATHLETES
Club/Activities participants DO NOT NEED SPORTS PHYSICALS!

The IHSA Sports Physical Forms needed to participate can be Download by Clicking HERE
Students must have all of the above 4 pages of IHSA physical forms completed and uploaded, just 1 or 2 pages of the forms uploaded is not a complete physical and will not be accepted.
Students Must have a valid sports physical examination within the last 12 months on file in order to participate in sports!
(PLEASE DO NOT BRING FORMS INTO THE SCHOOL OR ATHLETIC OFFICE - THEY MUST BE UPLOADED!)
Please note No other forms but the above IHSA ones will be accepted as a valid physical, immunization forms are not a valid sports physical.
